This had persisted<br \/>\nfor the past 3 days.<\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #0000ff;\"><span style=\"font-size: 24px;\">I had tried the usual remedies: cranberry juice,<br \/>\nd-Mannose, and even\u00a0GSE &#8211; but none of them<br \/>\nworked. Now, I needed to see my GP.<\/span><\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #0000ff;\"><span style=\"font-size: 24px;\">My GP took me right away, did the urine culture, and<br \/>\nsuggested I get started on antibiotics right away.<br \/>\nI wasn&#8217;t convinced. This inflammation<br \/>\ncan trigger a cascade of changes that can\u00a0damage multiple organ systems,<br \/>\ncausing them to fail.<\/p>\n<p>&nbsp;<\/p>\n<p style=\"font-size: 16px; font-family: Helvetica, Arial, sans-serif; color: #111111; margin: 0px 0px 12px; line-height: 22px; -webkit-text-size-adjust: auto;\"><span style=\"font-size: 18px;\"><em>If sepsis progresses to septic shock, blood pressure drops dramatically,<br \/>\nwhich may\u00a0lead to death.<\/em><\/span><\/p>\n<p style=\"font-size: 16px; font-family: Helvetica, Arial, sans-serif; color: #111111; margin: 0px 0px 12px; line-height: 22px; -webkit-text-size-adjust: auto;\"><span style=\"font-size: 18px;\"><em>Anyone can develop sepsis, but it&#8217;s most common and most dangerous in elderly\u00a0people or those with weakened immune systems. Early<br \/>\ntreatment of sepsis, usually with antibiotics and large amounts of<br \/>\nintravenous fluids, improves chances for survival.<\/em><\/span><\/p>\n<p>&nbsp;<\/p>\n<p style=\"font-size: 16px; font-family: Helvetica, Arial, sans-serif; color: #111111; margin: 0px 0px 12px; line-height: 22px; -webkit-text-size-adjust: auto;\"><em><span style=\"font-size: 18px;\">Many doctors view sepsis as a three-stage syndrome, starting with sepsis and progressing through severe sepsis to septic shock. The goal is to treat<br \/>\nsepsis during its mild stage, before it becomes more dangerous.<\/span><\/em><\/p>\n<p>&nbsp;<\/p>\n<h2 style=\"font-size: 21px; font-family: Helvetica, Arial, sans-serif; font-weight: bold; color: #54585a; font-style: normal; text-align: left; margin: 0px 0px 12px; display: block; letter-spacing: normal; line-height: 25px; -webkit-text-size-adjust: auto; padding: 0px;\"><span style=\"text-decoration: underline; color: #ff0000;\"><em>Sepsis<\/em><\/span><\/h2>\n<p>&nbsp;<\/p>\n<p style=\"font-size: 16px; font-family: Helvetica, Arial, sans-serif; color: #111111; margin: 0px 0px 12px; line-height: 22px; -webkit-text-size-adjust: auto;\"><span style=\"font-size: 18px;\"><em>To be diagnosed with sepsis, you must exhibit at least two of the following symptoms:<\/em><\/span><\/p>\n<p>&nbsp;<\/p>\n<ul style=\"font-size: 16px; font-family: Helvetica, Arial, sans-serif; color: #111111; margin: 0px 0px 12px 24px; line-height: 22px; -webkit-text-size-adjust: auto; padding: 0px;\">\n<li><em>Fever above 101.3 F (38.5 C) or below 95 F (35 C)<\/em><\/li>\n<li><em>Heart rate higher than 90 beats a minute<\/em><\/li>\n<li><em>Respiratory rate higher than 20 breaths a minute<\/em><\/li>\n<li><em>Probable or confirmed infection&#8230;&#8221;<\/em><\/li>\n<\/ul>\n<\/div>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #993300;\"><span style=\"font-size: 24px;\"><span style=\"text-decoration: underline;\"><strong>Why was I so reticent to take some antibiotics?<\/strong><\/span><\/span><\/span><\/h2>\n<p>&nbsp;<\/p>\n<h2><strong><span style=\"color: #993300;\">Although antibiotics are life-saving in many cases,<\/span><\/strong><br \/>\n<strong><span style=\"color: #993300;\"> they can also have tremendously serious side effects,<\/span><\/strong><br \/>\n<strong><span style=\"color: #993300;\"> like: \u00a0C. Coli!<\/span><br \/>\n<span style=\"color: #0000ff;\"> And the best antibiotic to treat it was: Cipro!<\/span><\/h2>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #0000ff;\">I was not happy about taking Cipro.<br \/>\nShe prescribed (horse-sized) Cipro pills for 3 days.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #ff6600;\"><span style=\"font-size: 24px;\">To prevent getting C. Diff, I started taking massive<br \/>\namounts of probiotics:<br \/>\nVSL #3DS, Floracor-GI, S. Boulaardi, and Culturelle.<br \/>\nI wanted to make sure to flood my colon with enough<br \/>\nvaried strains in huge quantities to prevent C. Diff.<\/span><\/span><\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #0000cd;\">I had to make sure to take the Probiotics 3 hours after the Cipro to ensure they wouldn&#8217;t cancel each other out.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<h1><span style=\"color: #ff0000;\"><strong>How would I possibly have gotten E. Coli?<\/strong><\/span><\/h1>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #0000ff;\">I started to re-trace my steps.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #0000ff;\">About 8 days\u00a0prior to, I had started my 10 year old<\/span> <span style=\"color: #0000ff;\"> ferrel cat on a new dry food\u00a0(grain free of course).<\/span> <span style=\"color: #0000ff;\"> It was expensive and healthy for her. But I noticed,<\/span> <span style=\"color: #0000ff;\"> after a day or so, she developed urinary problems:<\/span> <span style=\"color: #0000ff;\"> she was spending lots of time in the litter box,<\/span> <span style=\"color: #0000ff;\"> she was\u00a0straining and in pain, and I saw blood in her urine.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #0000ff;\">I felt badly for her, picked her up as I always did, and<\/span><\/h2>\n<p><span style=\"color: #0000ff;\"> caressed her silky back. She was clearly in pain.<\/span><br \/>\n<span style=\"color: #0000ff;\"> (And I didn&#8217;t realize I had an open cut in my finger).<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #0000ff;\">I took her to the Vet, and he confirmed she had E. Coli.<\/span><\/h2>\n<p><span style=\"color: #0000ff;\"> He told me some companies ( even good ones) have<\/span><br \/>\n<span style=\"color: #0000ff;\"> had issues with food contaminated E.Coli. He put her on<\/span> <span style=\"color: #0000ff;\"> some antibiotics for 7 days.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #0000ff;\">I called the store\u00a0where I had purchased her food,<\/span> <span style=\"color: #0000ff;\"> and told them about the E.Coli contamination my vet<\/span> <span style=\"color: #0000ff;\"> suggested. They issued\u00a0me a full refund and told me<\/span> <span style=\"color: #0000ff;\"> to throw out her food.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #0000ff;\">And a few days later, my symptoms started, and<\/span><br \/>\n<span style=\"color: #0000ff;\"> then increased in severity, pain and seriousness.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #000080;\"><strong><span style=\"background-color: #ffff00;\">So,<\/span><\/strong><span style=\"background-color: #ffff00;\">\u00a0<\/span><\/span><span style=\"color: #800000;\"><strong style=\"font-size: 24px; color: #0000cd;\">How would I possibly have gotten E. Coli?<\/strong><\/span><\/p>\n<h2><span style=\"color: #800000;\">The natural behavior of cats is to groom themselves alot.<br \/>\nMy cat was no different, and always groomed herself<br \/>\nthoroughly after eating. And when she contracted<br \/>\nE.Coli from the contaminated food, she groomed herself<br \/>\ntoo. And when I picked her up to caress her, I didn&#8217;t<br \/>\nknow she had E.Coli, and I didn&#8217;t realize I had an open cut<br \/>\non my finger.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #800000;\">And so when I caressed her, touching those areas where<\/span> <span style=\"color: #800000;\"> she had groomed herself with the contaminated E. Coli<\/span> <span style=\"color: #800000;\"> bacteria, I contracted the E.Coli bacteria from her<\/span> <span style=\"color: #800000;\"> through the\u00a0open cut on my finger.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<h2><span style=\"color: #800000;\">My cat was better within a few days.<\/span><br \/>\n<span style=\"color: #800000;\"> And I got better within 5\u00a0days.<\/span><br \/>\n<span style=\"color: #800000;\"> And thankfully, because of the preventive measures I<\/span> <span style=\"color: #800000;\"> took, I didn&#8217;t get the dreaded C. Diff.<\/span><\/h2>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #ff0000;\"><strong><span style=\"font-size: 32px;\">The moral of this story is: You can get E. Grant, PhD<\/h3>\n<p>(OMNS Jan 16, 2014) There were 3774 papers published in 2013 with vitamin D in the title\u00a0or abstract according to pubmed.gov, up from 3099 in 2012. Among the top 20 vitamin D\u00a0papers chosen to highlight for 2013 were 11 reviews, five observational studies, one geographical ecological study, one trial, one laboratory study, and one analysis of data from published results.\u00a0The papers were chosen in part by the number of times they have been cited in other works as\u00a0reported by Scholar.Google.com and in part based on expert opinion by vitamin D researchers.<br \/>\nThe fact that two-thirds of the papers were reviews is an indication of the relative maturity of\u00a0the field. However, as noted in the discussion, the weak link in the vitamin D story is the limited\u00a0number of randomized controlled trials (RCTs) reporting beneficial effects of vitamin D.\u00a0As discussed in the paper by Heaney [2013], the primary reason for this result is that vitamin D\u00a0RCTs have been poorly designed and conducted in general; researchers have generally designed<br \/>\nvitamin D RCTs based on the pharmaceutical drug model, which assumes no other source of the\u00a0agent and a linear dose-response relation. These conditions are not satisfied for vitamin D.\u00a0As a result, many of the trials enrolled people who had relatively high serum 25-hydroxyvitamin\u00a0D [25(OH)D] levels and gave them too little vitamin D to produce a beneficial effect.<\/p>\n<h3 style=\"font-size: 16px; font-family: Helvetica; font-weight: bold; color: #606060; font-style: normal; text-align: left; margin: 0px 0px 10px; display: block; letter-spacing: normal; line-height: 100%;\">Especially Important Original Studies<\/h3>\n<p>A randomized controlled trial with 400 or 2000 IU\/d vitamin D3 found that gene expression\u00a0in white blood cells was affected by vitamin D [Hossein-nezhad et al., 2013]. There was some\u00a0improvement in 291 genes and significant improvement in 66 genes. In addition, 17 vitamin\u00a0D-regulated genes were found with new candidate vitamin D response elements &#8220;which have\u00a0been shown to be important for transcriptional regulation, immune function, response\u00a0to stress and DNA repair&#8221;. This study provides strong support for the role of vitamin D in\u00a0affecting risk of cancer, autoimmune disorders and cardiovascular disease.\u00a0An observational study found that serum 25(OH)D levels below 30 nmol\/l (12 ng\/ml) were<br \/>\nassociated with 1.0-1.6 additional cases of spontaneous preterm birth before 35 weeks&#8217; gestation\u00a0per 100 births for nonwhite mothers compared to levels above 75 nmol\/l (30 ng\/ml)\u00a0[Bodnar, 2014]. Placental inflammatory lesions played a role in this relation. It was not\u00a0understood why there was no relation between serum 25(OH)D level and risk of premature\u00a0birth for white mothers. This paper adds to the literature on the beneficial role of higher\u00a0serum 25(OH)D levels during pregnancy.<br \/>\nVrieling et al. [2013] conducted a prospective cohort study in Germany including 2,177 incident\u00a0stage I-IV postmenopausal breast cancer patients aged 50-74 years. Lower concentrations of\u00a025(OH)D were significantly associated with an 86% higher risk of overall mortality and a 76%\u00a0higher risk of distant disease in stage I-IIIa but not in stage IIIb-IV breast cancer patients.\u00a0This study adds to the evidence that vitamin D plays an important role in cancer and all-cause survival.<\/p>\n<p>A laboratory study identified several genes associated with triple negative\/basal-like breast\u00a0cancer that are regulated by vitamin D receptors [Laporta and Welsh, epub], thereby providing\u00a0additional evidence that vitamin D can be useful in reducing risk of and treating breast cancer.<\/p>\n<p>A pair of papers published in the open-access journal <em>Dermato-Endocrinology<\/em> discussed\u00a0the evidence that solar ultraviolet-B irradiance and vitamin D reduce the risk of autism.\u00a0In one, the prevalence of autism by state for those aged 6 to 17 years was found inversely\u00a0correlated with summertime solar UVB doses [Grant and Cannell, 2013]. Rates for African\u00a0Americans were higher than for European Americans. The variation with respect to solar\u00a0UVB doses is similar to that reported previously for many types of internal cancer and\u00a0dental caries in the United States. Summertime solar UVB doses in the United States are\u00a0highest in the Southwest, lowest in the Northeast due to a combination of factors involving\u00a0surface elevation, aerosol and cloud levels, and stratospheric ozone amounts. In the second<br \/>\npaper, the evidence supporting the role of vitamin D in reducing the risk of autism and treating\u00a0those with autism was presented and discussed. The mechanisms seem to include effects on\u00a0reducing DNA mutations, anti-inflammation and antiautoimmune effects, and upregulating\u00a0glutathione [Cannell and Grant, 2013]. It is not clear from either study what the relative\u00a0contributions of maternal and infant serum 25(OH)D levels might be. These two papers\u00a0add to the understanding of the roles of solar UVB and vitamin D in reducing risk of autism\u00a0and could lead to a reduction in the epidemic of autism.<\/p>\n<p>One of the roles of vitamin D is the reduction of inflammation. In a study of morbidly<br \/>\nobese women, serum 25(OH)D levels were found inversely correlated with several indicators\u00a0of inflammation including high-sensitive C-reactive protein, interleukin-6 (IL-6), and\u00a0tumor necrosis factor-\u03b1 (TNF-\u03b1) after accounting for confounding factors including body\u00a0mass index (BMI) [Bellia et al., 2013]. This study adds to the literature connecting low\u00a0serum 25(OH)D level to increased risk of inflammation. This is important since inflammation\u00a0is an important risk factor for many chronic diseases.<\/p>\n<p>Another paper analyzed data from 21 adult cohort studies involving 42,000 participants\u00a0with information on BMI, serum 25(OH)D levels, and genes related to BMI and 25(OH)D.\u00a0The analysis found that each 1 kg\/m(2) increase in BMI was associated with a 4.2% lower\u00a0serum 25(OH)D level [Vimaleswaran, 2013]. The authors were also able to conclude that\u00a0changes in serum 25(OH)D level would not affect BMI.<\/p>\n<p>One of the concerns regarding taking vitamin D supplements is the possible risk of developing\u00a0kidney stones. An analysis of data on 2000 participants of the GrassrootsHealth.net vitamin\u00a0D testing program followed for an average of 19 months found no statistically significant\u00a0association between serum 25-hydroxyvitamin D and kidney stones (P?=?0.42) in the range\u00a0of 20-100 ng\/ml. However, higher BMI was associated with an increased risk [Nguyen et al., 2013].<\/p>\n<h3 style=\"font-size: 16px; font-family: Helvetica; font-weight: bold; color: #606060; font-style: normal; text-align: left; margin: 0px 0px 10px; display: block; letter-spacing: normal; line-height: 100%;\">Comprehensive Reviews of the Health Benefits of Vitamin D<\/h3>\n<p>Pludowski et al. [2013] reviewed the evidence of health benefits of vitamin D as one of the\u00a0outcomes of a vitamin D conference attended by over 500 people in Warsaw, Poland in\u00a0October 2012. The evidence reviewed was primarily from observational studies. They\u00a0found &#8220;Adequate vitamin D status seems to be protective against musculoskeletal disorders\u00a0(muscle weakness, falls, fractures), infectious diseases, autoimmune diseases, cardiovascular\u00a0disease, type 1 and type 2 diabetes mellitus, several types of cancer, neurocognitive dysfunction\u00a0and mental illness, and other diseases, as well as infertility and adverse pregnancy and birth\u00a0outcomes. Vitamin D deficiency\/insufficiency is associated with all-cause mortality.&#8221;\u00a0This paper provided the basis for recommending serum 25(OH)D levels of at least 30 ng\/ml<br \/>\n(75 nmol\/l) for Central and Eastern Europe.<\/p>\n<p>Hossein-nezhad and Holick [2013] reviewed the effects of vitamin D on the risk of chronic\u00a0illnesses, including some cancers, autoimmune diseases, infectious diseases, type 2 diabetes\u00a0mellitus, neurocognitive disorders, and mortality. The authors recommend a serum 25(OH)D\u00a0level of at least 30 ng\/ml. This paper has open access.<\/p>\n<p>The paper by Haussler et al. [2013] summarizes the molecular mechanisms of vitamin D,\u00a0with primary emphasis on the classical functions of vitamin D on intestinal calcium and\u00a0phosphate absorption and effects on skeletal and calcium homeostasis, but also discussion\u00a0of how the active form of vitamin D, 1,25-dihydroxyvitamin D, affects gene expression and,\u00a0thus risk of chronic diseases such as cancer, cardiovascular disease, and type 2 diabetes mellitus.<\/p>\n<p>The paper by Palacios and Gonzalez [2013] presents an overview of what is known about serum\u00a025(OH)D levels globally. Six maps are presented with data on vitamin D status (percent of\u00a0population50% for European countries, but are 36% for China and 91% for India.<\/p>\n<h3 style=\"font-size: 16px; font-family: Helvetica; font-weight: bold; color: #606060; font-style: normal; text-align: left; margin: 0px 0px 10px; display: block; letter-spacing: normal; line-height: 100%;\">The Actions of Vitamin D<\/h3>\n<p>The effects of vitamin D on gene expression are controlled by the action of the hormonal\u00a0metabolite of vitamin D, 1,25-dihydroxyvitamin D on vitamin D receptors (VDRs). A paper\u00a0by Carlberg and Campbell [2013] reviews the actions of the VDR in comparison with those\u00a0of other transcription factors in order to better understand the role of vitamin D on gene\u00a0expression.<\/p>\n<h3 style=\"font-size: 16px; font-family: Helvetica; font-weight: bold; color: #606060; font-style: normal; text-align: left; margin: 0px 0px 10px; display: block; letter-spacing: normal; line-height: 100%;\">The Role of Vitamin D for Specific Health Outcomes<\/h3>\n<p>The paper by Song et al. [2013] presents a meta-analysis of 21 prospective studies of type\u00a02 diabetes mellitus incidence with respect to serum 25(OH)D level at time of enrollment\u00a0in the studies. The studies involved a total of 76,220 participants and 4,996 incident type 2\u00a0diabetes cases. The second order fit to the data went from a relative risk of 1.00 at\u00a035 nmol\/l (14 ng\/ml) to 0.45 at 150 nmol\/l (60 ng\/ml). This analysis provides strong\u00a0support for the role of vitamin D in reducing the risk of type 2 diabetes mellitus.<\/p>\n<p>Girgis and colleagues [2013] published a comprehensive review of the role of vitamin D\u00a0on skeletal muscle. It has 51 pages and 390 references. It should be the standard reference\u00a0on the topic for years.<\/p>\n<p>A paper by DeLuca et al. [2013] reviewed the evidence from 225 papers for the role of\u00a0vitamin D in reducing risk of nervous system diseases: Alzheimer&#8217;s disease, amyotrophic\u00a0lateral sclerosis, autism, multiple sclerosis, Parkinson&#8217;s disease, and schizophrenia.\u00a0The strongest evidence is for multiple sclerosis. They note that evidence from many\u00a0types of studies finds that vitamin D plays a crucial role in cellular proliferation and\u00a0differentiation, neurotrophism, neuroprotection, neurotransmission, and neuroplasticity.\u00a0Given the importance and devastation of neurological diseases, more research on the\u00a0role of vitamin D in reducing risk of these diseases is warranted.<\/p>\n<h3 style=\"font-size: 16px; font-family: Helvetica; font-weight: bold; color: #606060; font-style: normal; text-align: left; margin: 0px 0px 10px; display: block; letter-spacing: normal; line-height: 100%;\">A Skeptical Viewpoint<\/h3>\n<p>One paper that has gotten considerable publicity is one calling into question the health\u00a0benefits of vitamin D. Autier and colleagues published a paper comparing vitamin D\u00a0randomized controlled trials to prospective studies based on health outcomes with\u00a0respect to serum[25(OH)D levels [Autier et al., 2013]. While they found reasonable\u00a0evidence from the observational studies that higher serum 25(OH)D levels were correlated\u00a0with lower disease and mortality rates, they noted that few randomized controlled trials\u00a0(RCTs) agreed with the observational studies. They also noted that inflammation is an\u00a0important aspect of many diseases, but that RCTs have not shown that vitamin D can\u00a0reduce inflammation. They proposed &#8220;the hypothesis that variations in 25(OH)D\u00a0concentrations would essentially be a result, and not a cause, of ill health.&#8221; As noted\u00a0with respect to the paper by Bellia et al. [2013], there is evidence that serum 25(OH)D levels\u00a0are inversely correlated with markers of inflammation. The vitamin D RCTs conducted\u00a0to date have for the most part been poorly designed and conducted as pointed out by\u00a0Heaney [2014]. Vitamin D RCTs have mostly been done using the pharmaceutical drug\u00a0model, paying limited attention to other sources of vitamin D, not seeking to enroll people\u00a0with low serum 25(OH)D levels, supplement them with sufficient vitamin D to raise their<br \/>\nlevels to those that have significant impact on health outcome, and also measure serum\u00a025(OH)D levels later in the study. The proper way to examine causality with respect to\u00a0vitamin D is to apply Hill&#8217;s criteria for causality in a biological system [Hill, 1965].\u00a0These criteria appropriate for vitamin D include strength of association, consistent\u00a0findings in different populations, temporality, biological gradient, plausibility\u00a0(e.g., mechanisms), coherence, experiment (e.g., RCT), and analogy. Not all criteria\u00a0need be satisfied, but the more that are, the stronger the case. A number of health\u00a0outcomes have been found to satisfy Hill&#8217;s criteria for causality for vitamin D including\u00a0many types of cancer.
